Schedule an appointment

The MSU Main Library has a Passport Acceptance Facility, located on the 2nd Floor West Wing. We also offer passport photo services. We provide a convenient location for MSU Students/Faculty/Staff and their families, as well as community members. Please read below for important information about applying for a new US passport or renewing one.

Fall Hours:

Passport Photos can be taken anytime the Hollander MakeCentral is open. Please see MakeCentral Service Desk Hours for more information.

The MSU Passport Office is NOT be available to WALK-IN APPOINTMENTS.

The MSU Passport office will be available Monday-Friday from 12pm-5pm and Sunday 2pm-6pm by appointment only. Please make an appointment.

You must make an appointment at least ONE business day before your appointment request in order to secure an appointment. We are unable to take appointment requests over the phone or on the same day.

Make an appointment. When thinking about how much time you need for an appointment please consider:

  • The number of people you will be scheduling at one time. Allow yourself extra time for processing if you have young children.
  • Time and date preference. We will do our best to accommodate your preference.

Required Documentation when Applying for New Passport

  • DS-11 Application
  • Proof of Citizenship
  • Proof of Identity
  • Passport Photo
  • Fees. Two separate payments are required for a new passport:
    • Application fee: payable to the Department of State. See the fees section for a listing of fees. May only be paid with a check or money order.
    • Execution fee: A separate payment of $35.00 per application is required for the passport facility. Payment can made by cash, credit/debit card, or a check made payable to MSU Library. .

Up-to-date information on the required documents can be found at travel.state.gov or we have forms available to fill out in-person. If filling out the form by hand, please print legibly with black ink only and do NOT use any whiteout on the form.

Documents required will depend on your circumstances. You may call the National Passport Information Office at (877) 487-2778 if you have any questions.

Adult Renewal

Most renewals cannot be handled by Passport Acceptance Agencies. You can renew your passport by mail if your most recent passport meets ALL five of these requirements:

  1. Is submitted with your application
  2. Is undamaged (other than normal "wear and tear")
  3. Was issued when you were age 16 or older *
  4. Was issued within the last 15 years
  5. Was issued in your current name (or you can document your name change with an original or certified copy of your marriage certificate, divorce decree, or court order.)

Items Required

  • DS-82 Application - Filled out in black ink only, printed single-sided.
  • Most Recent Passport - Meets the requirements listed above.
  • Name Change Documents (if applicable) - Marriage license, divorce decree, court-ordered name change document.
  • Photo - One 2x2 inch passport photo meeting the U.S. Department of State requirements.
  • Application Fee - payable to the U.S. Department of State, check or money order only.

*If your last passport was issued when you were under 16, you will need to "renew" with an adult form (DS-11) and we can accept that form, payment, documents at our location.

New Adult Passport (Age 16 and Older)

  • A DS-11 Application
    • Do not sign the application until the passport agent instructs you to do so.
    • This form can be completed online, printed from travel.state.gov, or we have forms available to fill out in person.
    • If filling out the form by hand, please print legibly and use only black ink. Please do not use any whiteout on the form.
  • Proof of Citizenship (one of the following)
    • Original birth certificate or a certified copy (no hospital certificates or photocopies)
    • Previous U.S. Passport
    • Naturalization Certificate
    • Certificate of Citizenship
    • Consular Report of Birth Abroad
  • Proof of Identity
    • Valid Michigan driver’s license
    • State of Michigan ID (also must have secondary identification)
    • For non-Michigan residents, please bring your driver's license and a Department of State's approved secondary identification. MSU student, faculty, staff photo ID is acceptable secondary ID.
  • Passport Photo
    • A 2x2 inch photograph taken in the last 6 months. The picture should be of the full face with a plain white or off-white background. Visit travel.state.gov for complete details.
    • MSU Library offers a Passport Photo Service for an additional $10.60 (includes tax) fee.
  • Fees (two separate payments are required for a new passport)
    • Application fee: payable to the Department of State. See the fees section for a listing of fees. May ONLY be paid with a check or money order.
    • Execution fee: A separate payment of $35.00 per application is required for our passport facility. Payment can made by credit or debit card, or a check made payable to MSU Library. No Cash will be accepted at this time.

NOTE: Applicants ages 16-17 years old must provide proof of parental awareness when applying. Examples include one parent accompanying the applicant or a signed statement of consent from one parent with a front and back photocopy of parent's ID.

New Minor Passport (Age 15 and Younger)

  • A DS-11 Application
    • Do not sign the application until the passport agent instructs you to do so.
    • This form can be completed online, printed from travel.state.gov, or we have forms available to fill out in person.
    • If filling out by hand, please print legibly and use only black ink. Please do not use any whiteout on the form.
  • Proof of Citizenship (one of the following)
    • Original birth certificate or a certified copy (no hospital certificates or photocopies)
    • Previous U.S. Passport
    • Naturalization Certificate
    • Certificate of Citizenship
    • Consular Report of Birth Abroad
  • Proof of Identity - The parents/guardians must present ONE of the following identification documents to the acceptance agent:
    • In-state, fully-valid driver's license
    • Valid or expired, undamaged, U.S. passport
    • Certificate of Naturalization or Citizenship
    • Government employee ID (city, county, state or federal)
    • U.S. military ID or military dependent ID
    • Valid foreign passport
    • If you cannot present one of the IDs above, or for more information, please see Identification
    • Please note: If you present out-of-state identification, you must present an additional ID.
  • Proof of Parental Relationship - You must submit documentation that lists the parent(s) or legal guardian(s) of the child applying for a passport. The following may be used to show parental relationship:
    • U.S. Birth Certificate (also evidence of U.S. citizenship)
    • Consular Report of Birth Abroad (also evidence of U.S. citizenship)
    • Foreign Birth Certificate
    • Adoption Decree
    • Divorce/Custody Decree
    • Please note: Some documents, like a U.S. Birth Certificate, show both U.S. citizenship and parental relationship. These documents must be originals or certified copies (not photocopies).
  • Proof of Parental Consent
    • Both parents must be present. If one or both parents cannot be present, appropriate documentation must be provided. Please visit children under 16 for more information.
  • Passport Photo

Lost/Stolen Passport Replacement

Lost Passport: We can process your DS-11 Application and other necessary paperwork if you have lost your passport. Visit the U.S. Department of State - Lost or Stolen Passports for details.

If you report your lost or stolen passport online, please print out the confirmation and bring it with your documentation.

Birth Certificate

An Original or certified copy of citizenship documentation must be mailed with your application to the U.S. Department of State. The Department of State Passport Agency will mail it back to you.

A certified birth certificate must meet the following requirements:

  • Full name of the applicant
  • Date of birth
  • Place of birth
  • Both parent's full names
  • Raised, embossed, impressed or multicolored seal of issue authority
  • Registrar's signature
  • The date the certificate was filed with the registrar's office. This must be within one year of birth or you will need secondary evidence of citizenship as well.

Passport Photos

We can take your photo as part of your Passport appointment for DS-11 applications, or you may bring one with you from another vendor. We are accepting walk-in passport photos at this time.

Photo Cost - $10.60 (includes tax for two photos)

Please do NOT wear:

  • white or camouflage shirt
  • strapless top
  • any type of uniform
  • glasses
  • headbands

Frequently Asked Questions

Absolutely. Any U.S. citizen may apply for a U.S. passport with us.

The time it takes to apply for a passport varies. Bringing all of your documentation, including a completed application, will reduce the time spent in the office. It typically takes approximately 20-30 minutes per applicant to apply if you bring completed paperwork.

Yes, you may request expedited processing when you submit your application. You will need to add $60.00 per application to your check or money order made out to the U.S. Department of State if you choose that service.

Routine processing takes 8-11 weeks and expedited processing takes 5-7 weeks. Processing times are subject to change. Please check the Department of State travel site for the most current information.

The MSU Library Passport Acceptance Facility is not able to process an application faster than expedited service. You may need to make an appointment at the Detroit Passport Agency. Every situation is unique and there are special requirements. See the U.S. Department of State page regarding options.

You may track the status of your application online. Please note that it takes 6 weeks after applying for your passport to be tracked online.

Yes. The Passport Agency will send your original proof of citizenship along with any other original documentation back to you in a separate mailing.

No. Every applicant must apply for a new adult or minor passport in person, including minor/infant applicants.
