The front panel has the title on upper half of sheet, and a paragraph discussing how scientists discovered the process of producing vitamin D through irradiation by ultra-violet rays, and the importance of vitamin D for health. The back panel discusses how the public will not be charged for adding vitamin D to bread, which amounts to a "$1,000,000 Gift to the American People." Address at bottom reads, "Bond Bakers General Baking Company 420 Lexington Avenue New York City." In small letters in lower right corner: "100M-1-34-Form 953-R."
